To access any of the forms, please sign in to your Stanford account, download the form to your computer, and open up that specific file.

• MCS/Data Science Requirement Inquiry Form: As of September 1, 2024, all MCS students should use this form when requesting a substitution or exception (whereas MCS students were previously asked to fill out separate forms and ask for approval from their faculty advisor). 
• Transfer Credit: If you would like to apply transfer credit towards a major or minor requirement, please complete the following steps:
  • Submit the Course-to-Course Equivalency  form. 
  • Once a decision has been made regarding your course (whether approval or denial), please complete the Request to Use Transfer Credit toward a Major/Minor Requirement form.
